Welcome to Soulful Care

At Soulful Care LLC, we make the admission process simple, transparent, and heart-centered.
Our goal is to ensure that every person entering our home feels respected, informed, and supported from the very first conversation.
We partner closely with families, case managers, and Community Services Boards (CSBs) to create smooth, person-centered transitions into supervised living.

 

Who We Serve

• 18 years or older

• Virginia resident

• Diagnosed with an Intellectual or Developmental Disability (IDD)

• Eligible for a DD Waiver (Community Living, Family & Individual Support, or Building Independence) or private pay

• Able to live safely in a community-based, supervised setting

• Interested in developing life skills, independence, and community engagement

​Our Admissions Process​​

1. Inquiry: Email us at soulfulcare3@gmail.com to learn more or make a referral.

2. Eligibility Review: We confirm DBHDS and waiver requirements.

3. Tour: Families and case managers visit our home and meet our team.

4. Assessment & ISP: We develop an individualized plan based on each person’s needs and goals.

5. Admission: Once approved, the resident joins our community and receives a warm welcome to their new home.​​​​

        (We welcome referrals from CSBs, support coordinators, hospitals, social workers, and families.)
